Our Vision
Our vision for the future of Jurassic Coast PCN is to create a patient care system that includes everyone. A system where member practices work closely together and in collaboration with health and social care, voluntary sectors, community groups and local people to deliver efficient and timely personalised care. To make the best use of available resources to ensure that everyone gets the right support, in the right place, at the right time.
With a growing population increasingly living a 24/7 lifestyle, there is strong evidence that health outcomes are socially determined and that the conditions in which people are born, grow, live, work and grow old influence their health. Our vision focuses not only on direct health service provision, but also on helping people to help themselves and their local communities offering extended access to new and existing services is very important in creating our vision of care.

Ambition
Our ambition is to provide a joined-up care system where everyone has access to the care they need quickly and effectively.
To achieve this, we will focus on four key themes: Prevention, Supported Self-Management, Education and Empowerment and New Models of Care.
Prevention
We will support people to live healthier lives, reducing their risk of developing health conditions in future. In addition, we will proactively identify those at risk of ill health and put early measures in place to ensure people live longer, healthier and happier lives.
Supported self-management
We will work in partnership with the individual to help them to understand the nature and impact of their health condition, empowering them to have the knowledge, skills, confidence and support they need to manage their condition effectively in everyday life.
Education and empowerment
We will provide people with the tools they need to understand when and how and when to access healthcare services, allowing well people to thrive and those that are struggling to be supported, so that overall the whole population benefits.
New models of care
We will streamline our care processes and pathways, creating opportunities for new, innovative and effective models of care to be trialled and adopted.
Jurassic Coast PCN Location
Jurassic Coast PCN is located in West Dorset, extending from Lyme Regis on the Devon border to Maiden Newton and Beaminster in the north of the County down to the market town of Bridport and its surrounding villages.
With its rolling countryside views dotted with quaint, stoned villages, UNESCO recognised Jurassic Coastline, and bustling market towns, West Dorset is frequently listed as one of most desirable places to live in the UK.
The PCN’s footprint is the same as the combined boundaries of its core member practices, as shown below in our PCN map.
What is a Primary Care Network?
A Primary Care Network is a group of GP practices that work together with community, mental health, social care, and voluntary services in the local area, to provide comprehensive primary care services to a defined population.
PCNs build on existing primary care services and enable greater provision of proactive, personalised, and coordinated health and social care for people close to home.
Over 99% of general practices are part of a PCN that sign up to the Network Contract DES, which details our core requirements and entitlements.
What benefits do Primary Care Networks have for patients?
- They offer a larger range of care services that well be close to patients' homes, as well as improved access
- PCNs work with a wider range of health and community services
- Patients will be able to receive support for a more complicated conditions, and will have access to the health and care services that can support them
- Patients will be able to have more of a role in making decisions around their own health and the care they receive
What can you expect from your PCN?
Additional healthcare professionals are being recruited to work on behalf of Primary Care Networks. Roles include Clinical Pharmacists, Ageing Well Teams, Mental Health professionals and Social Prescribing Link Workers. By introducing a greater skill mix, local people will get more access to the support they need to keep people healthy and independent.

The ultimate aim is to deliver better health outcomes for the local population, as well as a reduction in health inequalities.
We also have doctors, nurses and healthcare professionals working collectively to try to anticipate a patient's needs, to avoid getting unwell in the first place. This will be part of a wider campaign to make people aware of the importance of taking better care of themselves to avoid future health issues.
